@User this is in klayout but it's the same device. There are two orange shapes (met4) with a larger blue shape (met3) underneath. The met4 square is the top plate of the MiM cap, the large met3 shape is the bottom plate. You connect on met4 normally to try and balance the charge build up during processing which can easily damage the thin MiM dielectric.
But short answer: there are two met4 shapes, one for each terminal of your capacitor
