If the bulk of the cascode is connected to source then in the layout you'll need to use a separate floating well. For example two PMOSs in cascode would need separate nwells for each because their bulks are at different potential. Gets more complicated with NMOSs. In general, that's a pain to layout so I would stick to power rail bulks unless you really need to change because the body effect is too high
