Strangely the tap is inserted just beside the endcap cell and the well is cut in between _ anybody seen this - @Mitch Bailey

no. Hardeded blocks are there

Are your hardened blocks using custom standard cells? Is there 1 tap cell and 2 decap cells in your hardened block?

no. no custom std cells.... let me check the count

I'm trying to figure out why there is only one tap cell and 1 decap cell in one row and only 1 decap cell in the other row. Are you missing fill cells?

Making custom standard cells is pretty difficult. You have to know exactly what you're doing, and make sure that all of your standard cells abut all other standard cells in all orientations without generating DRC errors.
