The ESD devices have very specific layouts and are characterized only for those layouts, which is why there are no device generators; you would just need to use the vendor GDS for the device (some of which are in the I/O cells and have not been broken out into independent layouts).

So... where do I get those GDS?
and just.... how do I connect them? Like so? That's the only example I've been able to find with a single fet, and it doesn't make sense to me.

Often that's nothing more than an nFET (a large one) with the gate tied to ground.
